REWARDS Cards
For the past several years, the Cystic Fibrosis Support Network of Michigan (formerly MPDCI) has supplied the CF clinics with $5.00 REWARD Cards to be given out to children and young adults who are compliant with their CF clinic visits. Starting this year, however, individuals with CF will receive their REWARD Cards directly from CFSN (with a maximum of 4 cards given out per year).
To receive your REWARD Card after attending your clinic visit, notify CFSN by calling 1-800-365-9140, mailing (CFSN, P.O. Box 790, Troy, MI 48099), or emailing (info@mpdci.org). Include the following: your name, address, clinic attended, the date attended, and your 1st and 2nd choice of REWARD Cards – Walmart, Burger King, Target, Meijer, and McDonald’s, Best Buy.
